Our Office
New Patients
While we do accept new patients, we do prefer if you make an appointment. If you have an appointment, please arrive 15 minutes before your appointment to fill out forms or take a moment to download patient forms and complete them prior to your scheduled appointment. Bring your insurance card and a list of medications that your child is currently taking.
Complete payment for medical services and care must be promptly made at the time such services are rendered. We do understand that this may be difficult for families at times; therefore, other payment schedules could be arranged in advance if you call the office. We will file insurance only for hospital care and managed care plans that have contracts with my practice. Please retain copies of all receipts at each visit since we do not give out itemized or year-end statements.
